
Transitions
Transitions is about planning for the future for young people with a learning disability from the age of 14. This involves looking at the changes that will take place in moving on from children's to adult services.
Transitions is the period from the age of 14 to around the age of 23 to 25 when hopefully the plans made are happening.
Social Services are involved in Transitions and the planning process will involve the Child, the Family, the School attended, a Social Worker and a Transitions Care Manager.
