
Person Centred Planning and Working
More people with learning disabilities know about Person Centered Planning with many now saying that they want to plan their own lives.
Person Centred Planning - what it is and why does it matter to you.
'Person Centred Planning' means putting you at the centre of planning for your life.
To do things in a way that the you want and in a way that helps you to be part of your community.
When you are at the centre of planning you are the most important person.
Your plan would include your hopes and dreams, how you can work towards making them happen with the support you need from family, friends and carers.
This will include how to keep safe and healthy.
What would a Person Centred Plan do for me?
It would help you to find out what is really important to you.
It would help the people who support you to get to know you better.
It would help you to find out ways to work towards reaching your hopes and dreams.
What support is there to get a Person Centred Plan?
You will need someone to help you to 'sort out' and record your ideas.
You will need someone who can listen to what the important people in your life have to say about you.
You will need someone to help you to put all this information together into a plan.
Your plan could be on paper, video, tape or CD ROM or more than one of these.
Your Person Centred Plan will be different to other peoples!
The person who will help you with your Person Centred Plan is called a facilitator.

Person Centred Planning Facilitators
Facilitors must be trained to help people with their Person Centred Plans.
The Partnership Board provided funding for 20 people throughout the area to be trained as Person Centred Planning facilitators (people who can help you with your plan), this included a parent and a person with a learning disability.
Awareness training
Person Centred Planning awareness training has taken place throughout the Craven and Harrogate areas with more training planned for the future.
