Person Centred Planning would help you to find out what is really important to you. It would help the people who support you to get to know you better. It would help you to find out ways to work towards reaching your hopes and dreams. This will include how to keep safe and healthy.

 
PCP Photosymbol

In 2001 the Partnership Board led the way in Person Centred Planning by funding a Person Centred Planner. This work has been recognised by the County Council who have now established a 4 person Person Centred Planning and Bridge Building Team.
